how do i identify a graphic card

i got this graphic card its a agp type ATI card but i don't have the box or cd for drivers and wondering if anybody knows what type it is

it has a sticker with

s/n : 220237016907
p/n : 1028342300 000002

and another sticker CN - 00P767 - 44571 - 299 - 00WT
and a one with Rev. A01

on the adapter on the side of the card it says MIC E - G012 -02 - 1214(B)

I believe you are looking for a Radeon 7500. I was looking for one with the same part number and that's what i found. I found the driver on driverguide.com
