I have designed a form with specific fields that I want everyone to be able
to use and email back and forth. None of the fields I have added show up
when I use the new form and send it either to myself or anyone else.

Are you and all your recipients on an Exchange server? If yes, did you
publish the custom for to the Exchange Organisation Forms Library? Have you
bound all your custom controls to custom fields that you have created? And
finally, have you made sure that your form has not been accidently split, so
that there is a compose form and a read form, and all your custom fields are
on the compose form and nothing is on the read form?

Assuming that you are and did and did and did, this problem is generally
caused by not forcing the message to be sent using the TNEF wrapper. Not
surprising, as Microsoft has never informed the general user that there is a
TNEF wrapper and how to invoke it. It is a little technical, so it is
probably best that this education effort has not been attempted. But it does
cause problems.

To make custom fields retain their custom field data and force the use of the
TNEF wrapper, open the contact item for the recipient, (like yourself when you
send it to yourself), select the email address you will be using, right-click
it, choose properties. For versions earlier than OL2003, there is a check box
on the bottom of the properties panel that is labeled something like "Always
send to this recipient using rtf format". Make sure that check box is
checked. For OL2003, the mail format check box has been moved to a drop-down
list. Select the rtf option.
