I need help creating a personal health record that includes: physician's
name, date of visits, type of visits, treatment, diagnosis, and follow-up
care.

how to set up a database.

In essence you'll need a Table for each type of "entity" - real-life
thing, person, or event. You'll need tables of Physicians, Diagnoses,
Treatments, Visits, and (unless these are all your own records)
Patients. It's a bit beyond the scope of this forum to have the
volunteers design your entire system for you, but please feel free to
come back with specific problems!
