How do I create a form to automatically fill in system information

I want to send an email to every new employee when their exchange account is
created and automatically fill in system information. How do I create that
form and make it available in Outlook?
 
To get into form design mode, choose Tools | Forms | Design a Form.

To make a form available to everyone, publish it to the Organizational Forms library.
