How do I create a Facimile Cover Sheet?

I am trying to create a Fax Cover sheet so I may print copies for use. Your
help would greatly be appreciated
 
It sounds like Access may not be the best application to prepare a Fax Cover
template which you wil use to print copies ... but if that's what you want
to do, you might try creating a report which is formatted as a fax cover.

I have created a FAX Template using a report and then used a form to
generate the FAX Coversheet populated with some of the data captured on the
form.
