I am trying to create a form for all staff to use when wanting approval for a
document. How can I do this?

InfoPath might be a better environment in which to do this. This comes under
the general rubrick of workflow, and Outlook/Exchange is a very difficult
environment to use for this.

If you can wait for the next version of Office, Work Flow is supposed to be much
easier in the new SharePoint Portal Server.
