How do I conduct a on-line survey using Microsoft tools?

Hi
I want to conduct a survey on line for my new business. The respondents will
receive an introduction letter/e-mail with the description of my company and
aim of the survey. Then they will access and complete the survey on line.

How do I receive the results of the survey? Also I want to store the info
collected in a data base with names & contacts.
