I would like to always automatically bcc a gmail address on an outlook
calendar appointments when setting the appointment. This is not the default
outlook calendar so the calendar sync does not work well, but an invite does.
Any ideas?

The closest you can come is to include the address in the Resources address
field. You could also send the appointment to the address as an ics
attachment to a mail message.

Thanks Brian. Is there a way to always and automatically generate the mail
message with the .ics
attachment to a single address when an appointment is set ... without having
to manually send the
invite?

All the addresses on any one outgoing message are equivalent. You cannot make
Outlook behave one way for one recipient, but another for a different
recipient of the same message.
