How can I set up an Action Item Database in Access?

I am trying to set up a database in Access for Action Items. Has anyone done
one before? Any help would be appreciated.

I'm sure many have; it would be a rather straightforward application,
depending on just what you mean in your context by the term "Action
Items". Care to expand upon the term? and indicate what you've tried?
