How can I print my name on an event instead of Unknown?

When I print an event from my Outlook calendar, "Unknown" is printed at the
top, above the solid line. How do I format the application to print my name?
I don't remember how I've personalized my calendar on previous computers,
but I never encountered "Unknown" before.

What version of Outlook and where are your folders hosted (Exchange or PST)?
