How can I make an orginal temple?

I need to design a temple for a form to be completed each time training is
completing detailing each traing session. I want to be about to make the
document so that all user has to do is click on the field and enter the info.
I am not really sure where to create this document, word, access or ????
Help please

I presume you mean a "template" - there are some people who seem to
worship Microsoft software, but that's heretical! <g>

We'd need some more information about what you're trying to record
here, but it *sounds* like an Access database would be appropriate.
However, Access does not create "documents" - it creates databases,
which can produce printed reports listing selected contents of the
database.

Again, not sure exactly what you're trying to accomplish!
