How can I fill up a printed paper form?

I am manually filling up a serialized pre-printed form repetitively for each
items that I work on. I would like to automate this process by creating a
form that looks similar to the form I'm filling up but not print the form
itself i.e, print only the typed information on the fields of the pre-printed
form. Then I want to save the filled up forms for my future reference. Any
suggestion is appreciated.

First, ignore that ignoramus who posted the other response.
Are you saying you want to print only the information you are now adding by
hand to the preprinted form? That can certainly be done, but I wonder from
the way you have phrased the question whether you are familiar with how a
relational database program works. You will need to provide more details if
you are to receive an informed response. If you are unfamiliar with Access
you would benefit by reading an introductory book on the subject. A Google
groups search will produce some titles and recommendations.
 
Are you referring to Access, Word or Excel. this is an
Access newsgroup?

I hit the keys prematurely there. Yes I would like to be able to do it in
MSWORD.
