How can I design a survey form containg 32 questions , each with .

I am trying to design a survey database. I used the form and option group
command. The problems I have are:
1. I have to use 3 forms to contain all these contents
2. when I use the mouse to scroll down, I can't lock the button
3. I can't change the background of the form.

I am not sure if I made a correct design of the database, please help. Thanks
