how can I activate my checkbox in PPT so it will fill in?

I am developing a form in PowerPoint and want users to be able to complete a
form that has checkboxes. I was able to insert the Active X control
checkboxes, but I don't know how to activate them so they can be used. Has
anyone done that before? We are not posting the file to the web.
 
Just to be clear do you mean a form in the vba sense with some vba collecting
the data or a simple non vba slide with boxes that users can tick and then
save?

If the latter you dont need to activate the check box as it will check and
uncheck IN SLIDE SHOW VIEW.
