Help with Forms in Outlook

I created a form in Outlook, to use for answering standard questions, with
basic date, text and a couple of yes/no boxes. It looked great, but when I
sent it out, it looked just like a blank email. What did I do wrong? I
published it, and I could see the form on my end, and when I sent it to
myself. Help!

Wrong newsgroup. Ask forms questions in
microsoft.public.outlook.program_forms
