help request form???

hi everyone, new to this site but it looks like it could come in handy.

this is probly a bit of a funny request but

at work i need to set up a problem request form, i will place it in the
start menu and any staff with a problem can click on it fill in what the
problem is and hit send and it will automatically be sent to me does
anyone know how to set one of these up??

It looks like InfoPath - in MS Office - might do what you want:

BTW, this isn't a "site". You are reading Usenet posts which have been
