Here's my Outlook mail accounts:

MYSERVER = mydomain.dom POP/SMTP (default)
GMAIL = gmail.com POP/SMTP

I've setup my e-mail account at MYSERVER to forward all e-mails to my e-mail
account at GMAIL. Then I use MS Outlook 2003 to retrieve e-mail from GMAIL.

If I create a New message, Outlook correctly sends the message via MYSERVER.

However, when a message is sent to MYSERVER which is automatically forwarded
to GMAIL, and I retrieve the message from GMAIL, if I Reply to the message
it's replied and sent via GMAIL.

There's an infobar at the top of the message which says,
"This message will be sent via GMAIL"

I can override this each time by clicking "Accounts" and selecting to send
the message via MYSERVER, but doing this every single time is undesirable.

How can I set it up in Outlook so that ALL messages replied to are sent via
MYSERVER by default, regardless if the message was received from MYSERVER or
GMAIL, or forwarded from MYSERVER to GMAIL and received from GMAIL?

You can't, because Outlook replies to email using the account to which the
email was sent. The only option you have (AFAIK) is to change the account at
the time of sending, as you have noted.
