Having a graph in a form

Hello,

Im trying to make a form for evaluating my employees.
Their will be several questions asked and for each
question their will be a list box with poor, fair, great.
Then I would like to have another form where I enter the
employees Id and on that form I would see a chart of there
grades

Example:
Enter Employee ID_________

Fname:John Lnameoe

Question 1
Poor: ---------------
Fair: --------------------------------
Great: -------------------------

Question 2
Poor: ---
Fair: -----------
Great: -------------------

Question 3
Poor: --------------------------------
Fair: --
Great:
