It means either a piece of hardware has failed, or a
connection is loose, or both.

If it's recognized by the BIOS but not in Windows, it generally means the
hard drive has too many errors on it for Windows to be able to read it.

If it's not recognized by the BIOS, first check it has adequate power and
that the cables are secure both ends.

It can be caused by any of the following

Bad hard drive
Bad motherboard
Bad PSU
Bad cables

thank you - I just replaced the PSU and it still won't
work? Do you think the drive itself is bad?

Or check that your settings in the BIOS haven't been changed to a different
harddrive. Make sure your BIOS is set to auto-detect the harddrive. If it
detects correctly but still won't boot then you may have a boot record
problem.
