hard disk could do fdisk

I have a hard disk(40G MAXTOR4K040H2) and used it as a backup store.but
today there is some problem:I setup it in my computer(third device)as I used
to do.but this time I could not start my OS system .I test the disk as first
device and setup from CDROM.read driver c: error.then I do format and
fdisk.when do fdisk ,"verify the integrity "could not finish it cycled from
0 to 56 .I used PQ DM... and all this have no effect .then I do low level
format and I do not know what can I do after low level format ?any tips let
my know I will apperciate
