Grouping and Separating Option Buttons

I am trying to create a survey that can be completed electronically
I've been using the ActiveX control toolbox to insert check boxes, tex
fields, etc, but I'm stuck on the option buttons (aka "radio buttons")

I have a series of yes-or-no questions. In each case, I want the use
to be able to select one or the other but not both.

Instead, you can only select one option button from the entir
document. How do I separate them so that it "starts over" each time?

I am sure that I have seen Jay Freedman answer this question for you
somewhere else. Please do not multipost.

In the properties dialog for the radio buttons, assign a unique group name
for each group of the buttons that you want to be mutually exclusive.
