Group Header missing

I have a report that skips a group header. The group name is shown in the
query result when it's run, so it should be in the report. The report output
shows the detail for the group, under the previous group header, with no
separation or page break between groups as there should be. The data are all
there in the right order, just no group header. When I change the query to
return only that group, the header shows in the report. If I add one group
(or more), the header disappears. I've never seen this behavior and I'm not
sure why it's happening. Can someone give me a tip on how to fix it? thanks
 
What version of Access are you using?

The existence of a column in your query to group by does not mean that you
actually have the grouping in the report. Select the Sorting and Grouping
item from the Reports menu/ribbon. Then make sure that the field you want
to group by is in the dialog (or Grouping section for A2007). If not, add
it. Then, make sure that the option is selected that indicates to include a
group header.
 
I'm using A2007. I have the correct field set up as Group on 'X' with A on
top, by entire value, with a header section and with a footer section, and
keep whole group together on one page. Additional groups follow but this is
my primary grouping with all associated data below it. I tried changing the
"keep whole group together..." to "keep header and first record together..."
and "do not keep group together...". None of these settings caused the
missing header to display. I have all other headers showing with the
associated data, separated as expected. Just this one header is missing, and
the data is grouped under the previous header with that group's data.
