Good Websites for Form Examples

Does anyone know any good websites for form examples. I have built a custom
form for my work, but after months of staring at the same thing my
creativeness has left me and I feel like I am looking at the most boring form
in the world.

I was wondering if anyone knew any good websites for form layout examples?

Actually, I often find that good looking form becomes just so familiar..that
you don't think it looks good anymore!!

Beauty is much in the eyes of the beholder....

Dear Albert:

Well, if you do put a collection of forms together, I'll be one of the first
to look at it! I've often thought it would be great to see a collection of
forms from different developers, to gather ideas and see examples of good
design. Imagine if all of the regulars here would send a screen shot of one
of their forms they felt was particularly functional or attractive (or
both!), and we could put them into a collection! Wouldn't that be great?

I wonder, however, if that would be a little like "giving away" proprietary
content...
