Good example of a quetionnaire database

Hiya

Does anybody know where I can find a good example of a database set up to enter questionnaire results and provide analysis on it (trends etc)?

My questionnaire has tick boxes on it with questions such as
Were your meals hot? _Yes _No (Please tick
How would you rate the service? _Poor _Satisfactory _Good (Please tick

I'm not bad with VBA and stuff, but have never really used Access for inputting data. A good example to use as a guide would be really helpful

I hope you might direct me somewhere handy
