Is there an optimum configuration guide for placing FSMO roles on certain
servers - which do you combine and why? Thanks.

Enkidu gave you some pointers but note the follow simple points:

1) PDC Emulator must be available/reachable by all other PDC
Emulators of that forest, all DCs of the same domain, and all
(potential) Master Browsers of other subnets -- this means centrally
on your net.

2) RID Master (usually with PDC Em.) must be accessible to the
other DCs of the same domain.

3) The Infractructure master must not be a GC (in a multi-domain
forest) and it must be able to find a GC.

4) Schema Master and Domain Naming Master are supposed to
be together (but I have never pushed the issue to find out the
downside if not.)

5) You need at least one GC (more for fault tolerance or performance)
available in every site.

I think this is just a recommendation because they're the forest wide roles
and are rarely used.

I've not read of any issues, gotcha's, whatever concerning these roles being
on differing servers.
