FrontPage98 "Forms" Tutorial Link Please?

Hi,

Don't laugh! ... Yes, I'm still using FrontPage98 because it still works
great for me and I only do my own website.

I finally need to create a SIMPLE "request form" (you know - the visitor
inputs their name, address, phone, etc.), where:
1) I receive the form contents in an e-mail, and ...
2) The visitor is sent a "confirmation" e-mail stating I received his
information, as well as get sent to a "thank you" page after they hit
"submit".

Would someone be so kind as to direct me to a "How To" forms tutorial link
for FP98??

Also, do I have to upload any FP98 file(s) or create any folders on my
server for this to work?? ... for example, something like a "cgi-bin" folder
equivalent??

Is your site hosted on a server that has the FP extensions?

I'm pretty sure it has the FP extensions, and if not, I'm sure they can be
installed because my server uses the Windows Server2000 operating system.

Keep in mind, I am strictly an amateur ...

But assuming it does, do you have a link?

Ok, then just look under Help for Forms. You will need to contact your web host to make sure the
extensions have been configured to process FP forms.

Thanks, I'm on-hold right now with tech support.

Also, I run my own dedicated server, so I can do, or have done just about
anything necessary to make this work.
