FrontPage Error. FP Email Form Problem

After generating a .htm page using the File>New menu in
FP2000, I am still under problems when specifiying an
email address for form results to be sent.

In particular, www.pennbrothers.com/xxx.htm

As you can see I keep getting FrontPage Error.

What do I need to tell my ISP??

p.s. by the way the site was done in FP2000..what do you
think of the site??
 
The site needs the FP Server Extensions Installed and Email Transport set up by the host
