Outlook from .dotx to .oft

Hi everyone, newbi here, in ms office 2007 i made a nice word form, with date picker and drop down boxes.
I would like to use it via oulook in some emails . My problem: how do you convert the word .dotx into a usable outlook .oft????

Hello again, If my question is too cryptic? i uploaded a jpg with extra information.
A long time ago i got a word template whitch i put into outlook to use as a email template (don't ask me how i did it, so frustrating that i don't know anymore how it got from a .dotx into a .oft). Anyway, the names in the dropdowns have to be edited. I have a new edited word template. So i can do one of two things 1/change (overwrite or is it write over?) the .oft or 2/ edit the dropdowns in the excisting .oft .
IN BOTH cases i don't have a clue how to do it.
Please, who can help me solve this problem?
