Frequent account lockout

I can launch outlook one time. If I close it out and
launch it again I get prompted for a user id and
password. When i try and enter it it doesn't work
because my account has been locked out. Anyone ever see
anything like this.

What version of Outlook? What operating system are you running it on? What
kind of server are you connecting to? What does your system administrator
say about that?
