Free Setup Installer

Are there any free setup / installer type apps available ?

Ideally with similar functions to Setup Factory 5 / 6 ??

With the ability to create a form, that the user can fill in when
installing the program, and the details from that form can be appended to
the shortcut created !

I've never heard of Setup Factory, but I figure you're talking about
a standard, run of the mill installer package. Anyway, here's a couple
that are worth their weight in gold:

Inno Setup http://www.jrsoftware.org/isinfo.php

NSIS http://nsis.sourceforge.net/

SpoonInstaller is the only free one I know of.
