Free Form Filler?

Is there a free program that will enable me to fill out a pre-printed form
on screen (presumably by scanning it first), load the original form into the
printer and then print just the entered data on it without printing the
scanned image too?

I've tried using FillOutaForm but that prints the scanned image as well as
the entered data - not much use when only the original is most likely to be
accepted.

Once a time upon, back in the days of DOS and dot-matrix printers, there
used to be a form-filling program that worked by entering the co-ordinates
and text for each field and, when you hit the PRINT command, it printed
out the sheet. The co-ordinates were accurate to the finest positional
resolution of the printer - something like 1/216th of an inch IIRC.

I wonder if there is anything of that nature around today (appropriately
updated for today's technology, of course)? If there is, I could almost
be tempted to <shock, horror, gasp> PAY for it.
(I will now probably be crucified for heresy.)
