FP 2003 Forms, Results not being saved to list

I've created a registration page at, http://stasmo.com/Registration_Page.htm
My confirmation page works, the email notification works but no data is
written to my form results at, _private/form_results_onlineregist.csv. Ideas
for fixing this problem??

I had this problem one. I always set the forms to save to both TXT and CSV.
Don't create the CVS file with notepad. create it with excel then upload it.
Not sure why but that fixed the problem for me.
