Good day. This is my first post in Word, so if I'm in the
wrong place, please advise.

I consider myself an above average user of Word, however,
I have no experience with forms.

I have an application that requires that the first page of
a document be a form containing a logo and several
blocking lines, separating the various components of the
document.

The users have been creating the first page without the
use of the form, although allowing spacing for the
compnents of the form and then printing it out by
inserting a printed form containing the logo and lines
into the printer and having Word print the text onto the
form.

We are at a point now where the various documents need to
be put onto a CD for the purpose of distribution to the
members of the group.

Is there a way to scan in the blank form and then be able
to use the blank form as a master for page one of each
document and have the entire form print on plain paper?

If this is possible, then I could then simply convert the
entire document into a PDF file and combine all the
documents into a book in PDF.

Scan the form and use it as a background image in the front page header of
the form. However, it would be better if you merely added the logo and
created the dividing lines using Word tools - such as borders to create the
appearance you wish.
