I am using some modified templates for form. Sometimes it
works, sometimes not ! I have even tried to go through
exactly identical and simple procedures of creating a new
form from the template, but with different (OK and not OK)
results. The joker again !

When you say modified, how did you modify them?
Can you provide a URL to the forms that work and those that don't?
