Forms. Reccomend a good site for teaching?

Hi all.

I've recently been diagnosed as a diabetic. "So what?" you correctly
say, but I'm trying to write a speadsheet where I can keep track of my
sugar levels, meds, food intake etc. What I want, in a perfect world,
is a button on the top of the worksheet that opens a form. I can enter
the info I want, and it gets put onto the next blank row when I press
OK.

I've googled for excel forms but so far I've not seen what I want.
I've seen how to add to the quick access bar, I've found stuff I don't
comprehend, but no tutorials of the type I need. So, can anyone help
please?

I have 2003 at work, 2007 at home and I want to put the sheet onto a
usb stick so I can keep it up to date. So I suppose it's got to be
2003 format.

What you're asking is a lot of details, maybe someone else will supply you with
a template, but have a look at this website > Section Excel and check
UserForm.Maybe that could be modified to your needs.
