I have created a basic form (currently Published in Public Folders)for
the user to enter 6 fields and email it to me. On test, when I edit the
form all fields are present but when I run it the Message screen
appears with all fields present, I enter my email address--> "Send" and
on receipt all fields are missing.
Also I can see the form when editing but if I go to Public Folders in
Outlook the folder is empty.
 
Two possible explanations:

1) You didn't put the fields on the Read layout of the form, only the
COmpose layout.

2) The form isn't published in your Personal Forms or the company's
Organizational Forms library or in your Inbox. A message form needs to be
published to one of those two locations in order for Outlook to have a form
definition to use to display the form. It doesn't know to look in some
public folder.
