You can create the forms in Word or Excel (or even PowerPoint), then save the
file.
Open up a new presentation, click Insert > Object.
Select Create from file.
Click Browse and look for the file.
Click Ok. You can have it display as an icon or a preview image.
Once done, click Slide Show > Custom Animation.
Click Add Effect > Object Actions > Edit/Open.
By doing so, the file will open during slide show view and users will be
able to save the form.
You can also make use of animation trigger so that the file will only open
when you click on the icon. Check the tutorial for more information:
