Forms from Outlook 2002 to Outlook 2003

When I installed Outlook 2003, all my custom forms seems to have
disappearded. When I went to Personal Folders to update the form, it
was not there. Do I need to recreate them in Outlook 2003.

Where were the forms published? If to the Personal Forms library, did you switch PST files when you upgraded?

How did you migrate to Outlook 2003? Did you use an import for your
messages? When you use import all the custom views and forms are gone.

I created the new PST file for O2003. I had multiple PST files
totaling around 5GB so Iwanted to take advantage of using one PST file
with the newmaximum size. I create the new PST file and moved the
contacts from the old PST file into the new PST file. The forms were
published in the Personal Folders.

Sounds like you didn't republish the forms in the new PST file. You will need to open Outlook with a profile that uses the old PST file as the default store. Create an item with each custom form, then save it as an ..oft file. Then restart Outlook with your new profile using the new PST file, open each .oft file and publish it.
