formatting form fields for dates and phone #'s

I'm creating a form using word 2000 (windows xp). I alway
use tables to creat the form, is this ok?

also, when I create a form and put in a form field, is
there a way to format it for certain criteria

ex.
phone number field have the ( )

date have 00/00/00

I when into the form field properties, but didn't see a
section to add the // or () any help is greatly
appreciated. :)
 
The date is easy, double-click your text field, click the "Type" dropdown
and select "Date". In the "Date Format" box type "mm/dd/yy". There are
others available from the dropdown to choose from if you want.
Unfortunately, there isn't the same type of "input mask" for phone numbers.
You could go into the properties for the phone number, click "Add help
text", then type in your desired format for the user, but they would need to
know to look at the bottom left hand side of the Status Bar for the help.

Actually, phone numbers are easier than dates. Format the text form field as
Number, and you can use the "Number format" to define a mask.

Table is fine if it works for you.

Double click your date field. In the type window, select Date. In the date
formt window, type MM/dd/yy

Double click your phone number field. In the type window, select number.
In the number format window type (###) ###-####
 
And how is that format (input mask) supposed to be typed in the "Number
format" text box? I fixed one up with "(000) 000-0000" and it gave me junk.
I am assuming you are willing to share how this format is supposed to be
assigned?

I have something similar in a form (000-000-0000), and regardless of what
numbers users input (e.g., 251-928-9999 or 2519289999 or (251) 928-9999 or
251/928-9999 or 251.928.9999 or whatever), it comes out as 251-928-9999.

Strange, I can get Greg's to work with the # signs but I can't get yours to
work. I tried (000) 000-0000, and (000-000-0000), and (000 000-0000) and it
always comes up with something weird like 2545551212-0000. Oh well, the
pound sign version works and even puts the parentheses around the area code.

You guys keep learning me stuff and I will have to knock something out the
other side! <VBG>

I was mistaken. I was looking at the locked form, not the Form Field Options
dialog. I had chosen 000-000-0000 as the display text. The actual number
format is ###-###-####. Sorry.
