Form

Hi

I have a form which all works fine goes to a database and brings up the
confirmation page. As soon as I include a File Upload box the confirmation
page doesn't come up it just goes to the form again.

Any ideas would be great?

Did you set it up w/ the permissions on a Windows Server and host that supports it as per
