The form needs to be published to the Organizational Forms library, with the "Send form definition with item" box unchecked. See http://www.outlookcode.com/article.aspx?id=61

For now i have it saved as a document as i am waiting for approval to publish
it. As for the check box you are referring to, I havent seen that one yet.

Thanks guys! Corporate just approved my form to be published and now it
works like a charm. Now I cant wait for us to get 2007 so there will be
templates for forms!

It is a good thing to ask, as one would not find it at a casual glance. InFor now i have it saved as a document as i am waiting for approval to publish
it. As for the check box you are referring to, I havent seen that one yet.
Where is that located at?
design mode, look on the properties page. In the left-hand column, right under
the set password box. You can't miss it.
