Form Text Fields in Headers?

I'm trying to create a narrative fill in form page which has a title box at the top (header?) which contains a graphic logo. Once the form is finished I want to be able to fill in a text field in the box, and have it show up in the box on all the following pages without having to rekey it. I can't get it to take a text field in the header, so what is a work around?
 
Use a cross-reference to the bookmark of the form field that contains the
desired data. Make sure to check the box for "Calculate on exit" in the Form
Field Options so that the cross-reference will update automatically.
