Form sumission leads to different pages?

Is there a way to make a form lead to different pages based on the response
given? For example if the responder writes 2 it goes to one page, but any
other response takes them to another. Thanks for any help!
 
Yes, this is possible when using custom server-side scripting (ASP, ASP.net, PHP) to process the
form submissions.
