form submission problem

I have a form whose contents move fine to the email address listed, but when
I add a file name and location using the private folder I get the front page
extensions aren't loaded or see your webmaster message. This will sound
stupid but what good does the browse button do next to the file location box
in the form properties window if there is nothing to select. Is there a way
to create a csv file ahead of time to browse and select?
 
Yes, just create a empty text file with the .csv extension.

You need your host to support the file upload component, and the Server
Extensions configured (by your Host) to allow this.

When you press "Browse" you can go to any folder you like on your hard drive
to select a file to upload to your server space.

Not sure what you're asking, but you can put a default value in the "File"
field.
