form problems

Ok, I am at the point I feel like a complete idiot with no return. I am doing
a form on my website in Frontpage and I just can't get it to work... I am
willing to take any advice here please help me!
The form is located on
http://www.centraldogsupply.com/youthhorse/start.htm
I want it to be emailed to me in a form that would look very simular to the
page as it exist now... PLEASE HELP.. I can't get it to do anything for me.
I use frontpage 2002
 
1. The link returns a page not found.
2. The FrontPage form only returns form results in lines of text, with the form field
name followed by the data entered by the user.
You can't make it return a "web page" so to speak.

That said, make sure you publish your form using the http:// address of your web site.
Also, make sure the email address used is one associated with your domain.
example: (e-mail address removed)

Also, make sure your hosting company has enabled the "forms to email transport" for your
domain account.

Along with what Steve already explained, if you wish the form to submit to
you in email via FrontPage, you will need to change this page to .htm or
..html extension and set the email and optional file in form properties.
