Form problem

I am new at creating forms with Word. I have created a form that I need but
when I enter text in one form field, the next fields move further down the
document. I want to keep the fields in place.
 
The usual way to prevent this happening is to set up your form as a table
with fixed cell dimensions.

Insert a table, turn off its borders if desired, set the cells to
fixed sizes, and put each field in its own cell.
