Form Problem

I have a basic form (http://www.hallssalvage.com/ask.htm) created in FP2003
that emails me the results and everything's been working just fine for
several years but all of a sudden when you hit submit you get the following
error message:

FrontPage Error.
User: please report details to this site's webmaster.
Webmaster: please see the server's application event log for more details.

from the following webpage:
http://www.hallssalvage.com/_vti_bin/shtml.dll/ask.htm

I've gone in and deleted/reinstalled the extensions but I still get the
error, and I can't seem to find this server application log anywhere in my
site. I've asked my host but they are being a bit slow to respond so I
thought I'd check in here. Anyone have any ideas?
 
The host has to review the log file, it is not in your web site space.

the only options in my control panel are uninstall/install/recalculate.. and

sorry, I forgot to put in my original post that after I uninstalled and
reinstalled the extensions I also republished the site and this did not
correct the problem
 
1) Open the form in code view and make sure you don't have duplicate webbot code
In the online one you have U-File repeated
U-File="_private/form_results.txt" S-Format="TEXT/PRE"
S-Label-Fields="TRUE" B-Reverse-Chronology="FALSE"
U-File="_private/form_results.txt" S-Format="TEXT/PRE"
2) test if it is the SMTP server by removing the send to Email option
- if it works w/o Email have you host Enable Email Transport
3) test if it is the _private permisssions by removing the send to File option
- if it works w/o Save file have your host check the permissions on the _private folder
