Form Post stopped working

I have a basic "Contact Us" form on my website that collects data input from
a user and emails it to me. Recently, it has quit working and I cannot
determine the cause or solution. When you click "submit" you get the
following message:

User: please report details to this site's webmaster.

Webmaster: please see the server's application event log for more details.

Nothing is entered in the web log or the application log.
I've tested SMTP - working
I can publish to the site.
Other Front Page Extension components seem to be working (photo gallery FP
ext?)
Permissions on the _private folder haven't changed
tried placing the results_form at the root - same result.

I'm absolutely stymied - HELP!
 
Have your host check / reset the FrontPage Extensions

This is on my internal SBS 2003 server - not a hosted site.
To insure FrontPage extensions are working properly on SBS, what is the
appropriate next step?
 
More info:

I've added a second page with a very basic form to test with.
I've tried it with form to file in _private without an email address and
tried it with an email address and no form...same result.
 
From IIS MMC or directly from the Server Extensions Admin run a server health check.

Thomas - I'm obviously missing something - I don't find a Server Health
utility/option anywhere. This is on an SBS 2003 Server - that make a
difference?
I see in other reading that Server Health check was an option in previous
versions - is Server health check available in SBS 2003 Premium?

sorry - I feel a bit clueless here...
 
Server Health Check is a function under the FP2002 extensions, if you have the FP2000 extensions it
is available as Check Extensions (I think) on the All Task menu for the specific web site.

Finally found it - I have to say, that particular little valuable utility was
a bit buried...

And this FIXED THE PROBLEM! Thanks for your quick feedback and assistance!

I followed your line of discussion except that I use a remote server
(POWWEB). I have gone to their site and uninstalled FPextensitons and
reinstalled but still me contact me form does not email me. I worked fine
until June 24 and then suddenly stopped. I did not change me form or do
anything to the site to make this function stop. I am at a loss as to what to
try next. Should I reinstall FrontPage 2003? Thanks for any help.
 
Have you contact your web host to see if they have made changes to their servers that would cause
your forms to stop working?
