Form page text box

My site has a page with a list of 300 parts. When a user clicks on a
particular part a form page will open in order for the user to request that
part. What I would like is to have the part the user click on display in a
form field on the form page. For example the use clicked on a washer in the
list, on the form page I would like the name of the part (washer) to appear
in a form field name Item.

I know enough about html to get me in trouble so if you could please advise
what code is need and in what pages the code should be placed I would be
grateful.
