Form options

I have created an order form at

I can't seem to figure a few things out. What I would like to have is
multiple drop down options. For example...let's say the customer chooses
Folded Note cards from the Product List drop down menu. I would then like it
so that another drop down list appears with all the different types of Folded
Note Cards. And the same for Artwork. If they choose Girls from the Artwork
category then another drop down menu appears with all the different types of
girls artwork.

Also, customers can place up to 4 different orders. Can I make it so that
Order 1 is open so they can fill out the information with the remaining Order
2 - 4 sections greyed out unless they click to add another order? Maybe a
radio button next to Order 2, 3 and 4. And unless they click on the radio
button the sections are greyed out.

I hope I am explaining what I need clearly so you can understand. Any
assistance would be greatly appreciated!
